What you need to claim?
In order to submit a claim, you will be required to submit the following documentation:
- Completed Smartlife claim form
- Certified copy of death certificate
- BI- 1663/BI-1680 or DHA- 1663/DHA-1680 form (forms completed by the doctor on death)
- Certified copy of the deceased ID
- Certified copy of the claimant ID
- Copy of police report for unnatural death
- Claimant’s bank statement
